1. Download & Print: Click Here To Download Club Donation Form. 

2. Select Product:  Utilize The Drop Down Menu & "Add To Cart" The Items You Wish To Donate.  Values You See Will Be The Donation Amounts To The First Tee.

3. Print: After Adding Donation Items To Cart, Select The "Print Quote" Button.  Print A Copy Of Your Donation Summary & Send It In With Your Completed "Donation Form" & Golf Clubs To The Address On The Donation Form. 

 *Upon Receipt Of Your Product, A Donation Will Be Made In Your Name To The FirstTee. You Will Receive A Tax Deduction Form For Your Donation Within 3 Weeks.

**If donation is made to a First Tee Chapter, 80% of proceeds will go to chapter and 20% will go to The First Tee Home Office.

No credit will be given for golf clubs if they have any of the following features:

• Iron Sets that do not have at least 5 consecutive clubs. Iron sets must also have a Pitching Wedge in them and all shafts must match (no non-matching reshafted clubs qualify)
• Clubs with dents, cracks, broken shafts, poor re-shaft work, visible shaft wear, counterfeits or any other features that would directly affect the product's resale value.
